Companion Hospice and Palliative Care of South Texas, LLC, based at 3201 Cherry Ridge Drive in San Antonio, says it will voluntarily stop providing hospice services effective November 6, 2026. The agency said it will end its Medicare provider agreement on that date and permanently close all hospice operations.
According to the notice, dated October 22, 2026, patients and their authorized representatives have been informed of the planned closure. The agency said it will work with other qualified hospice providers to arrange discharge or transfers and support continuity of care.
The agency listed a contact person, Tabitha Flutur, for questions about patient transitions, medical records or the closure.
